Therapeutic drug monitoring (TDM) is a clinical procedure in which patients' dose regimens are made to be as effective as possible by measuring a patient's blood at predetermined intervals to maintain a consistent concentration of a particular medicine. TDM is mostly used to monitor pharmaceuticals with limited therapeutic ranges, drugs with noticeable pharmacokinetic variability, drugs whose target concentrations are challenging to monitor, and drugs with documented therapeutic and adverse effects. Most medications do not require Therapeutic Drug Monitoring. The TDM procedure is founded on the notion that the dose and the plasma or blood drug concentration, as well as the concentration and therapeutic effects, have a measurable relationship.
